Pancreatitis necrotizante infectada: resultados en una cohorte de baja mortalidad

Objetivo: Analizar los factores pronósticos asociados con mortalidad en pacientes con pancreatitis aguda severa.
Métodos: Cohorte retrospectiva, pacientes consecutivos entre 1995-2004 con datos demográficos, clínicos y quirúrgicos, el diagnóstico de pancreatitis aguda severa definido como APACHE II > 8 o Ranson > 3.
Resultados: Se incluyeron 64 pacientes. El origen más frecuente fue colelitiasis (51,6%). La mediana del APACHE II=13. El promedio de estancia en la UCI fue de 16 ± 14,8 días (rango 1 - 63), la estancia hospitalaria total fue de 28,1 ± 19,8 días (1 - 84). El 82,8% de los pacientes fueron operados, el promedio de re-laparotomías fue de 4,2 ± 4,4 (1-18). El 81,3% desarrollaron necrosis: infectada o absceso=84,9% y estéril=26,4%. El 81,3% de los pacientes se manejaron con antibióticos durante catorce días. El 60,9% de los pacientes presentaron cultivos positivos en abdomen, los gérmenes más frecuentes fueron Enterococcus fecalis y Estafilococcus aureus. La tasa de mortalidad global fue 21,5% y la de mortalidad en pancreatitis aguda severa necrotizante infectada fue 28,9%. Los factores asociados con muerte fueron: SDRA (5,98, IC95%1,3-31, p<0,02), shock (24, 2,8-671, p=0,002) y APACHE II> 20 (16,2, 3,2-37,9, p<0,0007) estadísticamente significativos. No se encontró asociación con presencia de fístulas, infección, necrosis, uso de antibióticos y procedimientos quirúrgicos. El análisis de regresión logística mostró que shock y APACHE II > 20 fueron predictores de muerte (OR: 31,25(2,7-345) y OR: 1,27(1,07-1,51) respectivamente, p<0,05.
Conclusión: La presencia de shock y APACHE II fueron predictores independientes de muerte. La mortalidad global y la mortalidad por pancreatitis aguda severa necrotizante infectada fueron bajas: 21,5 y 28,9% respectivamente.
